After a daylong clash with Republicans over a proposal that has been a Democratic goal for years, lawmakers voted 220 to 215 to approve the $1.1 trillion, 10-year-plan that Democrats said would provide relief to Americans struggling to buy or hold on to health insurance in light of escalating costs and industry practices.
