The "Mayday" Pain Report authored by Dr. Russell Portenoy & Dr. Lonnie Zelter is supported by many prominent doctors and institutions.
I suffer from RSD/CRPS without proper pain management, which has now spread from my left arm to both legs. This could have been sent into remission, if diagnosed & treated early on. I now require a cane and/or a wheel chair on the "Bad Days". Crying & seizures daily as my family watches helplessly.
The RSDSA reports:

“Epidemic” of undertreatment affects more than 70 million Americans suffering from persistent back pain, headaches, joint pain; disproportionately affects minorities and low income."
The impact of the under treated & misdiagnosed has cost Americans more than $100 billion dollars annually.
